PREPARING

FOR YOUR APPOINTMENT

Prior to attending for your appointment please ensure the following:

Bring 2 Valid IDs

Bring two pieces of valid government-issued identification with you to your appointment. One of the two must be a photo ID (ie. Driver’s Licence, passport etc). Please note, Health Cards are not considered valid identification for our purposes. See a list of acceptable Identification to the right.

Do Not Sign Document

Do not sign any document that needs to be signed in the presence of the notary public or commissioner. You will sign the document in the presence of the Notary Public/Commissioner.

Arrive On Time

Please ensure you arrive on time for your appointment.

FORMS OF IDENTIFICATION

For your appointment you are required to bring with you two (2) pieces of government issued identification; one from each list below.

First ID

The first piece of identification must bear a photo of the holder. Examples of acceptable forms of photo issued identification include:
  • Driver’s License
  • Passport
  • Permanent Resident Card
  • Citizenship Card
  • Ontario Photo identification Card

Second ID

The second piece of identification may be any document that bears the full name of the holder. Examples of acceptable forms of a second piece of identification include:
  • Credit Card
  • Canadian Birth Certificate
  • SIN Card
  • Government Issued Marriage Certificate
  • Nexus Card
  • U.S Passport Card
  • Certificate of Indian Status Card
  • Student Authorization/Study Permit
  • Employment Authorization/Work Permit
  • Visitor Record
  • Temporary Resident’s Permit
Please note HEALTH CARDS are NOT considered valid identification.
